Suggest Treatment For Persistent Pain In The Testicles, Urethra, Bladder, And Lower Back After Frequent Masturbation And Holding Ejaculation

I m single 26y, i used to masturbate frequently and holding ejaculation for 6 months i stop , about one month ago the pain was at the tip of penis, now i have pain in testicles, penile urethra, urinary bladders area, lower back pain, pelvic ultrasound: normal prostate, residual volume of urinary bladder 160cc, but retrograde and MCUG was normal; ie. the bladder fully emptied , normal urine tests, scrotal uss: vas deferens congestion, grade1 lft varicocele, hotness beside left testicles, in NSAIDs, antibiotics for 1.5 month, very slight improvement or maybe no improvement at all, I m discomfort and in pain!?, what is the diagnosis and treatment plz?
