Suggest Treatment For Persistent Vaginal Bleeding

I have had my period non stop for nearly 6 months now, it stops sometimes for a day or two then starts again. Sometimes it is heavy and sometimes light, it also goes from bright red to really dark. the last two days it has been really heavy and alot of clots. I am 28 and had my third child about 13 months ago, I dont know if that has anything to do with it. Should I worry? Is there anything I can do to stop my period?
