Suggest Treatment For Red Swollen Area On Ankle

friend wont see doctor.its an area on the inside left ankle and foot thats red and swollen with white flakey edge around red swollen area.a thick one or two inch scab in the center of area on ankle above ankle joint.the area is 8 to 9 inches vertical and 4 inches or width of ankle horizontal its tight shiny. swollen out 3 quarters to an inch and ascending out in color on edge from red to purple before flakey edge.the area is painful to touch and pain goes to knee joint.from start 2-3 months going with this.said it started small.he is 49 yrs old his complexion is pale white with freckles an irish family trate and says he has an iron issue with rust spots from pea size to half inch in places .he believes that it was a recurring rust spot and got worse.can you please help so i can find him medication.he sleeps outside in his arizona room and i think he was bitten by a spider or insect .i researched diabetic ulcers.looks similar.its not brown recluse or black widow .might have been an assassin or kissing bug.i minor in entomology. he has loads of money he is a veteran cobra and apache rotor mechanic for 101st airborn with no insurance. he will loose his leg if cant figure it out or knock him out and drag him to my doc.
