Suggest Treatment For Severe Anal Itching

Hi,

I have been having MAJOR anal itching. The itching is not only the anus, but the cheeks as well (and it goes a good distance out). It is only at night, though, and is so bad that I get kept up at night. I have heard mention of pinworms, but wouldn't pinworms just affect the anus? My cheeks are red and rashy, a little raised and scaly. What else could it be? Thanks
