Suggest Treatment For Severe Back And Shoulder Pain

I suffer from back problems and have lots of pain. I have had 2 previous surgerys. My problem now is I get get a bad back pain and when it hits and both shoulder blades hurt and it makes me sick to my stomach and I fell like I am going to throw up. I was getting ready for chruch this am and it hit me and I started with the dry heaves. I ws wondering what would cause that.
