Suggest Treatment For Severe Bruxism

Hi I suffer from severe bruxism, my dentist has given me a mouth guard to wear when I sleep and my doctor has recently put me on trepiline 10mg but I still seem to grind, which is causing alot of health problems like earache, migraines, tense neck and shoulders and severe toothache. Is there anything that I could do to stop t grinding? Its making me really miserable
