Suggest Treatment For Stage 4 Kidney Failure

I live in Florida and am seeing a kidney doctor regularly for stage 4 kidney failure. I will be in New York for 5 months starting May 1st. My doctor says I should see a kidney doctor there. I'm concerned my insurance, Humana, may not cover me there. Should my primary physician refer me to doctor up there?
