Suggest Treatment For Swollen And Reddish Penis Head

Hi im 17 and uncircumcised. For some reason my penis doesn't look anything like the typical uncut penis, yet the foreskin seems tight and the tip is narrow and swollen red. However it has always been like that so Iv never deemed it as a serious problem. Also when I urinate, there is still pee that leaks. Could you explain?
