Suggest Treatment For Testicle Pain

I am a 31 year old male. In the last 10 years I have been experiencing this extremely pain/pressure under my testicles. A pain then shoots to my anus. My penis gets this really hard errection and I get heat flashes in the process of this happening. My anus tightens up and I feel this painful pressure. And I feel dizzy and like I will pass out and I have shortness of breathe, this last for about 10 to 15 minutes and then it goes away and it happens like once a month or once every 2 months....what is this?
