Suggest Treatment For This MRI Report

I don t know if I really need rotator cuff tear surgery. range of motion is 170 degrees, forward flexion, 160 degrees of abduction with pain in the abduction arc, internal rotation to T8 posteriorly in midline, external rotation 60 degrees. Positive impingement sign. Positive Hawkins sign. Positive apprehension sign. Rt should MRI evaluation demonstrates a moderate-sized retracted full thickness tear of the supraspinatus tendon. I believe it might get worse after surgery. What do you think?
