Today Morning When I Check My Blood Sugar On Random,,

Today morning when i check my blood sugar on random,, the reading shows 233. But when i check on last oct. on random, the reading was 120. This is first time i am saw this variation. Should i start medicines or not. Usually i took less sugar with food. But how it got increased this much... little confused... Please advise me.
