Two Weeks And A Half Ago Y Menstruation Came Back

Two weeks and a half ago y menstruation came back after 4-5 years, but since then I have been lightheaded and weak. Today I notice some blood discharge again. I am under identical hormones therapy but I forgot to change my estrogen patch twice already. Would this be the cause of it? Thanks!
