What Are The Itchy Hard Black Sores On My Skin?

I have suffered from this years now. I have been clean off of crack for a year and half my skin has sores and they ictch and something hard and black comes out this black thing keeps slowly coming out and I know when it's out Feeling like I'm going crazy
