What Are The Symptoms Of Attention Deficit Disorder?

I ve always had symptoms of ADD, but I never paid too much mind to it until now. In the recent years (past two years) my grades have been dropping to an extreme, I forget things mid-sentence or from conversations for minutes ago, and I can never get myself to focus on my task no matter what. Should I get diagnosed for ADD? What happens after that?
