What Cause Pain While Pulling Back The Foreskin?

I have some questions about phimosis.When I try to pull my foreskin back, I can see the urethral opening and an area of small diameter.Should I be concerned leaving the opening exposed? Will it become dry and painful? Because when I try to pull the skin back up, I feel a quick sharp pain for some reason.
