What Cause Ringing Sound In Ears?

I have a constant ringing and swishing sound in my ear and its been there since I was a child. Recently after having my wisdom tooth removed the ringing became louder and there's a rattling (like chains being flung around) sound in my left ear whenever I speak. I know I probably have tinntinitus, but what could the rattling sound be caused by?
