What Causes A Flat Red Splotch On The Buttocks?

my husband has a red splotch on his buttocks. It is about an 2 inches in total diameter with a 1/2 inch raw center. i thought it was ringworm but it is not round. He says it hurts. I can't find anything that looks like it online. It is not raised more flat. Any ideas what it might be??
