What Causes A Hoarse Voice?

I have had an increasingly hoarse voice for several years now. The hourseness used to come and go, now it is constant.

I am a smoker. I have, in the past been diagnosed with asthma, and was diagnosed with Type 2 Diabetes 9 months ago.
