What Causes A Penis Blister On Intercourse Or Masturbation?

Every time I have sex with my girl or I masterbate I start to get a blister on my penis. I stop and it ends up going away but it comes back the next time I do either activity. What is going on? I'm starting to get insecure about it and have stopped having sex with my girl and masterbating. Help!
