What Causes A Recurrent Sore On The Lip?

i have this sore on my lip that has been coming back annually for two or three years maybe longer, it usually occurs when i bit my lip. it gets hard and than the tissue turns soft and starts to almost soften up to the point where it peels off. i put abreva on it night and day and try not to touch my face nor do i put any lip gloss on it. what is this type of sore and how do i get rid of it? is there a possibility that the sore could be the HSV?
