What Causes A Throbbing Earache With History Of Goiter?

I was diagnosed with a large thyroid goiter when I was pregnant about 2 years, originally thought it was just an ear infection but my ears were fine. It still feels like I m experiencing ear pain but if I push on the nodule with my thumb I can feel the same ear sensation. What is causing the throbbing earache sensation? It comes & goes sporadically. I m not having any difficulty swallowing just this annoying sensation that comes and goes.
