What Causes Abdominal Cramps While On Mirena IUD?

I have been having severe cramps in my lower abdomen since last Friday night (similar to labor). The walk-in clinic has ruled out appendicitis and UTI but want me to get an ultrasound. I do have a Mirena IUD. Could it have perforated my uterus and could these cramps be symptoms?
