What Causes Abdominal Distension After Tummy Tuck?

I had a tummy tuck a year and a half ago. My upper abdomen was distended and the doctor performed liposuction to reduce the distension. I still have a distension that is very firm. A ct scan did not show undue scar tissue. I am not sure what avenue to persut to get to the bottom of this issue. Ideas?
