What Causes Abnormal Bowel Movement In An Infant?

My formula fed 5 week old is now having dirty diapers after ever feeding that are yellow and seedy (similar to a breastfed babies stool).The consistency has changed from what it used to look like and he's going much more often. Also has diaper rash. Is this normal?
