What Causes Abnormal Vaginal Bleeding?

I had bleeding for 4 weeks then stopped had an ultrasound 4 days after stopped bleeding which showed a cyst on one ovary and an endometrial thickness of 13mm. The day after the ultrasound I started bleeding again. I am 35, 2 children through fertility medication. This has been happening for years. I am worried about cancer.
