What Causes Air To Come Out Of Ear?

My boyfriend was jumped recently and ended up being punched in the right ear. The ear it self has some bruising however when he blows his nose air comes out of his ear. He recently went to the family DR and she said his eardrum was fine. Should he go for a second opinion? He still has air coming out of his nose and today he got water in it from the shower and he said it burned.
