What Causes An Itchy Nipple With Discharge?

I need help. I have this itchy left nipple with white discharge. what could be the causes of this? I noticed it in September, its not going away. i didnt see a doc. Im really worried now. it itch so bad. sometimes not..but once it attack, its really worst.
