What Causes An Urge To Pass Stool Frequently On Being Diagnosed With Paranoia?

Have an urge to poop fairly regularly, 6 times a day or more but nothing usually comes of it. Poop itself is not as hard as it once was and i have a constant paranoia of shitting myself. What can this be? I am fairly healthy but do not have the best diet. This has been going on for a month now
