What Causes Bad Breath?

My husband s breath smells like dill pickles or sour kraut. It makes me gag at times it s so intense. What could cause this? He is 67, has insulin dependent diabetes, is a 1 a day cigar smoker, weighs 260 lbs and has medically treated high blood pressure.
