What Causes Balanitis?

dear sir,
iam 32 year old & married 4 year before.iam suffering from balanitis 2 month before ,at that time i used antibiotic cream ,problem within 7 days.
Now it re-occour from 2 days.what treatment should i start?.which care should i take?why it re-occour?

note-i diagnosed balanitis from symptoms......

i will consult doctor but for sake of knowledge plz
