What Causes Black Outs Along With Tinnitus?

hello , for the past few days I have been boticing I have been blacking out. I hear a ringing in my ear , then everything seems like I m in a dream/ when I come to I have urinated on myself this has happened twice in the past 2 days... any ideas what this could be caused from?
