What Causes Bleeding Despite Being On Birth Control Pills?

Hi I have been on micro nor birth control pills for seven days, I am bleeding I went to the dr they did nothing. Prior to being put on these pills I had a vaginal ultrasound which showed no problem I however had heavy bleeding so they gave me provera which worked awesome but like I said now the bleeding is back, how long can I expect this bleeding to last
