What Causes Bleeding During Intercourse?

My question is: I have sex with one person and we use condoms I been having sex with the same male about 1 every 3 months I use condoms & lub. not all the time but most of the time I bleed during sex. I believe it is just lite and the lub makes it more obviouse.
