What Causes Bleeding During Urination And Penis Pain?

Hi sir. Im 24 years old guy...and when I go toilet I have red blood with no pain ...my head will pain and I feel hot ..what is the cause of that ? I feel scared when i saw too much blood in the toilet ..total red in colour ...before also it happened but after some day it will be normal ..im loosing my weight also please suggest
