What Causes Bleeding In 85 Year Old?

My 85yr old mother with dementia(she lives in a secure memory care facility) had blood that looked like a period in her Depends this afternoon. She told me about finding the blood earlier today. I was with her when she voided and then wiped and there appeared to be a very very minimal amount of old or dried blood on the tissue. She does have a history of hemroids and UTIs. She is on prescription Hyophen for the UTIs. Should I be alarmed? What should be my next step?
