What Causes Bleeding Through Penis During Sex?

After intercourse my husband went to the bathroom. He noticed blood on his penis and thought it had come from me. When he urinated blood came out of his penis and when I subsequently went to the bathroom I found he had left blood in me as well. What could have caused this?
