What Causes Bleeding When From Pens While Stretching Foreskin?

hi i am from mumbai my problem is that my penis skins slips back when i force it but in the tip of back side my skin is not seperate from the upper part it is joint to upper part due to which when i had sex my upper back side of penis started bleeding because the skin might have stretched please suggest me
