What Causes Bleeding While Passing Stools?

I bleed almost regularly when passing stool. It is not External Hemorroids It seems like internal ones. Fresh blood. Sometimes the skin breaks when wiping with toilet paper. Normally, no pain but bleeding can occur even with "normal" effort. Is my thin getting thinner?Are there some corrective actions?
