What Causes Blood, Bacteria And Epithelial Cells In Urine?

I have had a urinalysis positive for blood since 2005. No symptoms. Now I have same plus few bacteria and few epithelial cells. Blood co2 and chloride only elevated 2 above normal. Had ct with no results yet. Any ideas why I would have 3 tests positive for blood for a period of ten years?
