What Causes Blood In Urine Despite Having Treatment For UTI?

I went to the doctors nearly 3 weeks ago with what I thought was a uti. Back pain and lower abdo pain, dipped my urine as I work in a hospital first sample positive nitrates leucocytes and blood. Tried trimethoprim and didn't work. Went back a week later with same symptoms urine sample sent off and was given a week of amoxicillin. Sample negative for infection.Back again today with same symptoms. Urine showed that I was dehydrated and had blood in urine. I am going for an ultrasound of kidneys. Didn't really get any explanation of what dr though it might be. Any ideas?
