What Causes Blood Spotting Despite Birth Control Shots?

I am a 22 year old, white female. I have been on the DEPO birth control shot for the past 2 years, but I have missed my last shot. I have not had a period in two years, but lately when I urinate I see bright red along with what appears to be dried blood only when I wipe. I do have cramps in the lower pelvic areas. I do see spotting in my underwear. Can you help me on what this might be?
