What Causes Blue Spots On Tongue?

Our son is African and has blue spots on his tongue. He has had a high fever for four days and has OTB and is on an antibiotic. He has been tested for malaria but in USA must have repeat test. We have questioned malaria as well. Any idea what these spots could be from and should we be concerned. He does have a blister on his lips.
