What Causes Breast Pain With Swelling And Lump Sensation?

My mom is complaining of serious pain in her breast. It started Monday in her nipple, and has spread further above that area by today. She said that she feels a lump inside, and the pain is extremely severe. She wants to stick a needle in there, because she says she thinks it feels like it needs to be "popped". The skin is a bit red, but there is no scarring or marks like a bug bite or anything else. She also said that there are no "wrinkles" on her nipple like there should be. It is a bit swollen. The area is about 2 inches in length, and about 1 inch in width. Could this be a clogged duct? Could you please help with some sort of advice? She is hurting badly, and doesn't know what to do. She is concerned about going to the ER, being that it is just interns there. She is worried about having someone not specialized in the gynocological practice working on that sensitive area of her body. Thank you.
