What Causes Breathlessness While Having Acute Diastolic Congestive Heart Failure?

I was diagnosed with acute diastolic congestive heart failure in January after a 4 day stay in cardiac ICU. I am 62 years old and have difficulty breathing, somtimes even at rest. I was not referred to a cardiologist. I have a family physician. Should I have a cardiologist? Should I switch to an internal medicine doctor as well? Thank you!
