What Causes Burning Knotted Muscles In Back And Shoulder?

I get really bad burning knotted muscles in my back, and shoulders that will not go away with medication, massage, heat or cold. Pus an awful pain between my shoulder blades, what could this be? I had neck surgery on C3/4 C4/5 3 years ago, but have never had any other problems with the spine.
