What Causes Chest Pain,breathlessness And Shoulder Pain?

Last summer my 16 year old daughter started getting really bad chest pains, difficulty breathing, kind of lightheaded, and shoulder pain when she would exercise. It goes away after about 15 min of complete rest. When she was a baby she did have a mitral valve prolapse, but went away and she was fine. Could this be heart related? She was also very active and in multiple sports until she started getting these symptoms.
