What Causes Chest Tightness And Shortness Of Breath Leading To Insomnia?

I have been having chest tightness for more then two weeks with shortness of breath which worsens when i lay down accompanied by hot sweats what is wrong with me? I cant sleep which often turns into insomnia because im afraid i wont be able to breath while sleeping.
