What Causes Chronic Blood In Urine With Lower Back Pain?

Have had ongoing blood in urine for months, several infections and pain upper quadrant, right side and low back front aching. Now,have waited months for cystoscope as Catscan did not reveal kidney stones, though have had them in the past. Could it still be gravel in the kidney or am I looking at something more serious? Thank you
