What Causes Chronic Headache

My daughter has a chronic headache, she wakes up with it, goes to bed with it, sometimes wakes her in the night. She functions, but it seems to be there more than not. ENT came up with nothing, neurologist, same. Any ideas? This has been more than 2 months.
