What Causes Chronic Pain In The Shoulders And Back?

Hi well I been having back problems started after taking this osteoporosis medication. Well back was feeling good went back to work was doing physical therapy before anything well now its my upper part shoulder area is nasty in knots back to therapy today. How can I work with pain and osteoporosis is this considered a disability.
